Lucy Lebrett psychotherapy is committed to complying with the terms of the General Data Protection Regulation (GDPR) and to the responsible and secure use of your personal data.
This notice explains what personal information Lucy Lebrett collects and holds, and why I collect this data, how long it is kept and your rights over your personal data.
Information about you
I collect personal information from you when you enquire about my psychotherapy services in order to set up an initial consultation. This information includes contact details, your availability and other relevant personal information. When treatment commences your name, postal address, e-mail address, telephone numbers, details of your GP, psychological medication prescribed and details of any other relevant health professionals engaged in your care are collected and held. I would only contact your medical practitioner/s or others with your consent, unless I consider you or others to be at risk. Once a patient finishes psychotherapy, all data is stored securely for 6 years and then destroyed.
Use of your information
Your personal information will be used only to provide my services. I never give your details to third parties without your knowledge and permission, unless there is a legal requirement, if there is a child or adult safeguarding issue or a perceived risk of harm.
Protecting information
I make every effort to treat and store all information about you securely electronically, and password-protected. Communications with this service may be sent by e-mail. For ease of use and compatibility, communications will not be sent in an encrypted form unless you require it and give me permission to communicate with you in that way. E-mail, unless encrypted, is not a fully secure means of communications. Whilst I endeavour to keep my systems and communications protected against viruses and other harmful effects, I cannot bear responsibility for all communications being virus-free.
Your rights
You have the right to see any information I hold about you. You have the right to request that information I hold about you is deleted once your treatment with me has been ended. You also have the right to require that information I hold about you is rectified if it is inaccurate or incomplete.
